To the editor: Dr. Waitzkin (1) and his Marxists consider Third World physicians as belonging to the bourgeoisie and bemoan their defection to the First World. Should they not rejoice that the Third World bourgeoisie, robbed of its intellectual elite, will succumb earlier to the onslaught of the revolutionary proletariat? I can understand their sorrow that “imperialism reinforces international class relations and medicine contributes to this phenomenon.” But they would do well to ponder why the proletariat, contrary to Marx's expectations, has not broken national boundaries but rather is breaking heads in boundary wars—Russia versus China, China versus Vietnam, and
