To the editor: I am writing in response to an article by Rendell and associates in the February issue (1). It contains the following statement: “It is interesting that normal fasting and postprandial C-peptide levels were found before tolbutamide administration.” The authors then speculate that since C-peptide levels reflect endogenous insulin secretion in normal subjects, in their patient normal endogenous insulin secretion was occurring but that perhaps “an extra pancreatic factor” may have hindered the action of the endogenous insulin.
