Abstract

House of Welcome (HOW) Adult Day Services is a specialized program for persons with memory loss due to Alzheimer's disease and related dementias serving older adults and their families in Chicago's north suburban communities. Evaluation is a critical component of HOW's program. HOW staff members use observing quality of life in dementia, a reliable and valid observational tool, to assess the impact of structured activities on participants. Data from observing quality of life in dementia are computerized and a variety of reports can be generated for care planning, communicating with families, and reporting to funding sources and accrediting bodies.
