A 51-year-old woman with a history of splenectomy for traumatic splenic rupture as a child presented with right-sided abdominal pain. A pelvic and renal ultrasound (US) did not demonstrate a cause of the pain, although a complex left ovarian cyst was noted. A double-contrast abdominal and pelvic CT revealed multiple soft tissue implants within the peritoneum, raising the possibility of metastatic disease or, given the history of splenectomy, splenosis. SPECT images of Tc-99m labeled damaged red blood cells clearly showed multiple ectopic splenic implants.
