Following the description of positive factors in the long-term retention of corneal prosthesis, a brief analysis is made of the most frequent causes of implant extrusion. The purpose of this paper is to describe the prosthesis used in 12 cases. The implant is similar to that used in previous studies, with a slight modification consisting of an anterior flange. The operative technique has also been modified by the incorporation of a layer of Dacron mesh sutured immediately over the prosthesis, up to the insertion of the recti muscles. Periosteum or fascia lata is applied over the mesh. No extrusions have been observed in a 2− to 5-year period of follow-up.
