I am grateful to Dr. Sandroni for bringing to our attention once again the need to reform premedical education. He highlights the idea that I raised in my May 2008 editorial: that explicit and implicit requirements for admission to medical school, along with the way in which we use grades and test scores, are a strong—perhaps the strongest—influences on the choices students make during their premedical education. Although there are better forums in which to debate implementation details, I applaud his plea to explore innovative approaches to this formative stage of education.
