HISTORY - A 29 year-old man, formerly a water skiing national champion, participated in a two-days tournament in Buenos Aires, Argentina during fall, 1996. Five days after the competition he complained of lower back pain, myalgias in both calves, fever and headaches. In spite of being initially treated with NSAID, he persisted with fever, vomiting, low back pain and intense headaches. He was admitted after five days of symptoms.
PHYSICAL EXAMINATION - He was an ill man in no acute stress, with pulse 90 bpm, BP 110/60, Temp. 101 °F, low back pain with negative straight leg raising test and tenderness in both calves Neurologic examination was normal, and he has no lymphadenopathy, liver or spleen enlargement. In the following days he persisted with fever and presented oliguna, jaundice, conjuntival suffusion, tachypnea, renal dysfunction, anemia and thrombocytopenia. Blood and urine cultures obtained on admission were negative.
