An 18-year-old, world-ranked female swimmer presented with fatigue. She reported sleeping ten to twelve hours nightly for the past four months and never feeling rested. She had a case of pharyngitis three months prior to presentation which caused her to miss two consecutive weeks of training. At the conclusion of the previous season up to the day of presentation, the swimmer had taken a voluntary, extended one-month training break with no change in her symptoms. She denies fever, chills, night sweats, anorexia, and weight loss. Menses are normal. Rheumatologic review of symptoms was positive for oral ulcers, sicca symptoms, and Raynaud's phenomenon. Cardiac review of systems was positive for tachycardia to heart rate 180 with minimal exertion or at rest.
