This article describes an advanced breast cancer patient who underwent breast-conserving therapy (frozen section procedure) and primary reconstruction with a latissimus dorsi flap and later experienced a localized recurrence in the donor region. The recurrence was discovered 15 months after the initial diagnosis, after the patient reported severe pain symptoms. Until now, iatrogenic tumor cell transfer in breast cancer has only been discussed within the framework of needle manipulations. The exact risks are unknown, and to the best of our knowledge, the risks in primary reconstruction have not been described to date.
