Traditionally, access to oropharyngeal tumors is through a transmandibular approach, but such access can be very morbid. Chemoradiation has gained ground as primary treatment, but carries a high rate of dysphagia, dyshponia, chronic pain and xerostomia. Minimally invasive approaches provide the benefit of locoregional control without the morbidity of chemoradiation or a trans-mandibular approach. Minimally invasive resections create a reconstructive challenge because they often require free flaps for anatomic restoration and the inset requires suturing in a narrow, tight space with limited line of site. Trans-oral robotically assisted reconstruction is the logical solution.
