To the Editor:
We thank Dr. Rao and Dr. Shah for taking interest in our article titled Effect of internal limiting membrane peeling on the development of epiretinal membrane after pars plana vitrectomy for primary rhegmatogenous retinal detachment. Retina 2015;35:880–885. Regarding the first comment from Dr. Rao, we agree that both articles are indeed similar. However, we do not regard our study as a “replication.” Our study was presented in the Annual Meeting of Korean Ophthalmologic Society in 2012. Furthermore, we had submitted this article to “The Journal of Retina and Vitreous diseases” in December, 2013. Unlike Dr. Rao's study, which only compared the incidence of postoperative macular pucker (MP) and visual acuities between internal limiting membrane peeling group and the noninternal limiting membrane peeling group, we also analyzed other factors through various subgroup analysis (e.g., risk factors associated with MP development, visual acuity differences between the internal limiting membrane peeling group and the noninternal limiting membrane peeling group in all patients and in macula on retinal detachment patients, the mean time of epiretinal membrane development). Thus, we consider our study to be more informative.1,2
Second, we agree that the MP incidence after vitrectomy for rhegmatogenous retinal detachment has been reported to be higher in recent studies when compared with previous studies.1–6 As mentioned in the discussion section of our study, this could be because we considered epiretinal membrane to be present if a thin membrane was observed optical coherence tomography, even if it was not obvious in the fundus examination. As Dr. Rao also suggested, the higher incidence rate of MP may be associated with small-gauge vitreoretinal surgery. But, as Dr. Rao said, further studies need to clarify the factors associated with higher MP incidence after vitrectomy for rhegmatogenous retinal detachment.
