Our investigative team would like to thank de Vries et al. for calling the publication of Lopes et al. (1) to our attention. The cited study is a very important one in the overall care of patients posttransplantation and should be taken seriously by the transplant community. There are several published national guidelines that recommend “therapeutic lifestyle changes” as a first step in addressing cardiovascular risk in patients at high risk for cardiovascular disease (2–8). Such lifestyle changes include increasing physical activity, reduced fat diets, and cessation of tobacco use. The study cited clearly indicates that dietary measures are effective in lowering body weight and fat mass after kidney transplant. The combination of increased physical activity and prudent dietary habits after transplant should be adopted as routine care in this group of patients with high incidence of obesity and cardiovascular disease. Guidelines for “lifestyle changes” are easily accessible on the web at http://www.nhlbi.nih.gov/guidelines.
