Quadrupled semi-T anterior cruciate ligament (ACL) reconstruction offers advantages over the semi-T/gracilis and bone-tendon-bone constructs. Careful graft harvest and preparation, using tips presented here, ensures that graft length is adequate in almost all cases. The quad semi-T graft exhibits sufficient strength and minimal harvest morbidity. Combined with an aggressive rehabilitation program that emphasizes full early flexion, this procedure produces excellent clinical results.
