Excerpt
Faecal incontinence is a debilitating problem predominantly affecting posimenopausatl women. We hypothesised that it should benefit from hormone replacement therapy (HRT). We prospectively examined its effects on (i) symptoms of incontinence and (ii) anorectal function tests in a cohort of 20 postmenopausal subjects. Eighteen women (90%) experienced some improvement in symptoms, particularly with respect to incontinence of flatus, defaecatory urgeney and faecal staining. Five women were symptom-free at six months. There were no changes in anal mucosal sensation or nerve conduction, but significant changes in resting (p < 0.0001) and squeeze (p=0.04) anal canal pressures, and in maximum tolerated volume in rectal sensation (p=0.04) were noted. We conclude that symptoms of faecal incontinence can be reduced with HRT in postmenopausal women. reflecting an alteration in the integrity of pelvic connective tissues.
