Chronic ocular hypotony after filtering surgery using antimetabolites is one of the most frequent and severe complications of modern antiglaucomatous surgery. Since long-standing ocular hypotony may lead to papilledema and macular folds as part of the chronic ocular hypotony syndrome, several techniques have been developed to reduce the outflow of aqueous humor through the overfunctioning filtering bleb, including conjunctival compression sutures, resuturing of the scleral flap, remodeling of the filtering blebs using the neodymium: YAG or argon lasers, and the injection of autologous blood or trichloroacetic acid into the filtering bleb in an effort to partially close and decrease the volume of the bleb. 1–4 We describe the clinical outcome of a patient who underwent a new technique to reduce an overfunctioning filtering bleb.
