Oral fluconazole (3 mg/kg per day) and ketoconazole (20 mg TID) are more effective than nystatin suspension (SOR: B, small and unblinded RCTs). Gentian violet is also more effective than nystatin, but can stain clothes and skin and can cause mucosal ulcerations (SOR: B, small retrospective cohort study).
