EVIDENCE-BASED ANSWER

Although the risk of infection after medical abortion is extremely low, universal administration of doxycycline along with buccal (as opposed to vaginal) misoprostol is associated with a decrease in the rate of serious infections by up to 93% (SOR: B, large retrospective cohort study). The major expected adverse effect is vomiting (SOR: B, cohort study). Universal antibiotics are not required but are an option for women receiving a medical abortion (SOR: C, expert opinion).
