The treatment of Keratocystic Odontogenic Tumor (KCOT) of the jaws remains controversial. The aim of this study was to report the outcome of our conservative treatment protocol for KCOT.

We report a case of KCOT of maxillary sinus with unerupted second and third molars in 16 years-old female. Enucleation of the lesion and chemical cauterization with Carnoy's solution was done.

No recurrence of the KCOT has been observed for 5 years follow-up and is disease free.

Owing to its rarity, all forms of treatment for KCOT have been associated with propensity for recurrence. Complete enucleation with chemical cauterization may offer good prognosis in selected cases.
