Abstract

Objective:

hypothyroidism(H) is the atherogenic potential of women with hypertension. The aim is to learn the possibility of optimization of treatment of lipid disoders by levothyroxine replacement therapy(LRT) for women with hypertension(Hyp), abdominal obesity(AO) and hypothyroidism.

Design and Method:

147 women divided into 2 groups:1gr.-73pts (52,8 ± 2,1years)with H who were undergoing LRT; 2gr.-74pts (50,3 ± 1,7years) without LRT; Body mass index (BMI), TSH, freeT4, T3, lipids were determined along with intima-media thickness. All the patients received аtorvastatin (10 mg per day) and patients in 1gr received dditionalLRT (75 mg per day). Patients were observed at the beginning and for 6months of treatment.

Results:

patients of the 2 gr. had authentically large serum levels of total cholesterol (TC) and low-density lipoprotein (LDL) cholesterol, intima-media thickness. a risk factor for atherosclerotic cardiovascular disease. After 6months the women of the 1gr. on a background combination of LRT and statin had demonstrated the reliable decline of these indexes. The women of the 2gr. who were on аtоrvastatin background setting only had demonstrated a tendency to decline of TC and LDL.

Conclusions:

the lipid profile of women with a H,AO,Hyp is characterized by аtherogenic disoders, the enhanceable levels of TC and LDL, intima-media thickness. Therapy of аtоrvastatin for 6months is insufficient for the achievement of the target levels of TC and LDL,while setting of the combined therapy of LRT and statin optimizes efficiency of lipid lowering therapy allowing to attain the normal levels of lipids without the increase of statin dose.
