To study the relationship of serum leptin level (SL), soluble leptin receptor (sLR), as well as free leptin index (FLI) with insulin resistance in men with general obesity and visceral obesity.Design and Method:
The study included 139 men with general obesity (BMI>30). Group 1 - 69 individuals with thickness of epicardial adipose tissue (EAT) <7 mm, group 2 - 70 individuals with EAT ≥7 mm. All patients were defined insulin level, index HOMA-IR, serum leptin (SL), soluble leptin receptor (sLR) and free leptin index (FLI), calculated as the ratio SL / sLR.Results:
In the group 2, SL and FLI were found significantly higher than in the group 1 [18,6 (12,3; 23,5) inverse 12,3(8,9;16,8) ng/ml, p = 0,018] and [16,5(13,4;22,4) inverse 6,8 (4,5; 10,5) ng/ml, p = 0,03] respectively, while the level of sLR was significantly lower in the group 2 than in the group 1 (p = 0,028). A positive correlation between SL and HOMA-IR index, TG, LDL-c (r = 0,620, p = 0,01; r = 0,502, p = 0,02; r = 0,580, p = 0,01, respectively) and a negative correlation between the level of sLR and HOMA-IR index and TG (r = −0,334, p = 0,018; r = −0,528, p = 0,01, respectively) was found in the group 2. Also a positive correlation between level of FLI and HOMA –IR index (r = 0,260, p = 0,015), TG (r = 0,610, p = 0,02); LDL-с (r = 0,460, p = 0,01) and negative correlation between FLI and HDL-c (r = −0,410, p = 0,021) was found in VO group. A positive correlation between the level of sLR and HDL-c (r = 0,418, p = 0,017) was obtained in the group 1.Conclusions:
In VO group, The Level of FLI may be a marker of leptin resistance, as in our study it is positively correlated with insulin resistance by HOMA-IR index in VO group.
