To introduce a new procedure for the treatment of intractable cases of migraine. To eliminate the excessive vascular and nervous effect by ligation of superficial temporal artery and middle meningeal artery and severance of greater superficial petrosal nerve.A total of 10 patients with cases of severe migraine underwent the surgery. A follow-up of 2 to 18 years showed no recurrences. Among the patients, three were living and well for more than 10 years. With an extradural approach, the procedure is relatively safe and simple. It stands as a good alternative for the treatment of intractable cases of migraine.
