This article considers the future of the disease model controversy. A discussion of the reform movement and the contemporary disease model is contrasted with the orthodox disease view of addiction. In order to best understand the contemporary underpinnings of the disease controversy, a brief examination of disease in general and its associated metaphors is undertaken. This article also explores the relationship between the disease concept and clinical practice. Some predictions for the 1990s complete the discussion.
