In the setting of a symptomatic pseudarthrosis, the main goal of additional surgery is to obtain a solid bony fusion. Either posterior or revision anterior approaches can be utilized and are supported in the literature. Although patient-specific factors should be a main driving factor in surgical choice, the posterior approach has clear benefits over revision anterior surgery mainly due to its significantly higher fusion rates, which consistently approach 100%.
