A 42-year-old man with symptomatic severe primary mitral regurgitation was admitted to our institution for mitral valve repair through a minimal access approach. Left ventricular systolic function was normal and preoperative computed tomography coronary angiogram showed normal origin of the right coronary artery and the left main system, codominant circulation and only mild plaque disease.
