A history of previous clinical response, but poor adherence to oral therapy, sometimes makes an injectable depot antipsychotic drug the choice for management of psychosis during pregnancy. However, there is very little information on placental transfer for most depot formulations and, until now, none on the transfer of depot paliperidone. To gather guiding information for the care of 1 mother-baby pair during labor and delivery, we measured concentrations of paliperidone in maternal blood and cord blood, using a novel liquid chromatography-tandem mass spectrometry assay method with application to very small blood volumes.
