The etiology of coronary vasospasm in orthotopic heart transplant recipients is largely unknown. It has been reported anecdotally or in very small series of patients, most of them associated with coronary artery vasculopathy or allograft rejection in the setting of an acute coronary syndrome, malignant arrhythmia, or cardiac arrest. We report a case of asymptomatic coronary vasospasm in a cardiac transplant recipient.
