Revision of a Well-fixed Brimingham Hip Resurfacing Cup—Modified Use of Explant Device

Abstract

Removal of well fixed uncemented acetabular component of a hip resurfacing prosthesis can be technically demanding and can possibly lead to significant bone loss which compromises revision surgery. Explant system (Zimmer, Warsaw, IN) is used to remove uncemented conventional modular acetabular cups without significant bone loss but its use has not been routinely described for removing monoblock hip resurfacing components. We describe a simple modification of Explant system, placing a matching trial liner inside the acetabular cup to ensure adequate centering, which allows use of this device for removing large diameter resurfacing acetabular components without significant bone loss and at no additional cost. Bone stock remains well preserved to permit revision cup implantation.
