A technique is presented for osteochondral autograft transplant system harvesting of dorsal metatarsal head cartilage to repair a central osteochondral defect in the first metatarsal head. In this technically demanding procedure, cartilage and bone removed with an aggressive cheilectomy is used to replace damaged cartilage. Anecdotal results of several cases show good outcome and no further surgery after 6 years. The procedure adds another joint-sparing option for the patient with focal damage to the first metatarsal head cartilage.
